Innovative care for women at high risk of bleeding during delivery

15 December 2024, Doha, Qatar – Sidra Medicine, a member of Qatar Foundation, has become the first hospital in the region to successfully perform a high-risk cesarean section (C-section) in its state-of-the-art angiography suite. The procedure, a collaboration between the hospital’s Interventional Radiology (IR) and Obstetrics teams, marks a significant advancement in maternal care for high-risk pregnancies.

The angiography suite – a highly specialized operating room equipped with advanced imaging technology – provides the ideal environment for procedures requiring precise image guidance. For patients at high risk of bleeding during delivery, performing the C-section in this setting allows for immediate access to critical interventions like endovascular embolization.

Prof. Johnny Awwad, Chair of Women’s Services at Sidra Medicine, said: “Traditionally, IR teams have provided backup support for high-risk deliveries performed in the obstetrics operating rooms (OR). However, patient transfer times from the obstetrics OR to the IR suite are lengthy, especially when patients are unstable and we have to factor several safety protocols. This multidisciplinary collaboration between obstetrics and IR, is what makes a difference in successfully managing high-risk pregnancies. It truly sets our hospital apart in terms of offering women in Qatar, access to the most innovative therapies, expert teams and transformative care.”

Performing C-sections for high-risk patients directly in the angiography suite, can significantly reduce transfer times from the obstetrics waiting room to the IR from approximately 60 minutes to zero. This approach will allow for faster intervention if bleeding occurs.

Dr. Walid Mubarak, Interventional Radiology Attending Physician and Clinical Lead at Sidra Medicine, said: “For women at high risk of postpartum hemorrhage, this technique can be lifesaving. The angiography suite allows us to instantly access cutting-edge technology and perform minimally invasive procedures. We can quickly navigate the patient’s blood vessels with tiny catheters and precisely deliver embolic agents to stop bleeding. This minimizes blood loss and significantly improves the chances of a healthy outcome for both mother and baby.”

Sidra Medicine’s pioneering approach to addressing postpartum hemorrhage (PPH) offers a critical solution for one of the leading causes of maternal mortality worldwide. Affecting approximately 1-10 percent1 of deliveries, PPH is responsible for about 27 percent of maternal deaths globally. Sidra Medicine’s innovative use of an angiography suite for high-risk deliveries sets a new benchmark for maternal care.

Dr. Ashley Robinson, Vice Chair of Radiology at Sidra Medicine, said: “This achievement demonstrates the power of integrated care, which is central to Sidra Medicine’s approach. In addition to IR and Obstetrics, we have adult anesthesia and dedicated neonatal intensive care specialists present, ensuring seamless, coordinated care for both mother and baby, all within the angiography suite. This innovative approach enhances safety and care for women undergoing high-risk deliveries.”

Patients at Sidra Medicine who are considered at risk for bleeding during delivery are encouraged to discuss this approach with their obstetrician to determine if it is the right option for them. Contact us at 4003 3333 to learn more about our maternity services.

Done in collaboration with the Qatar Precision Health Institute (QPHI) and led by Dr. Khalid Fakhro, Chief Research Officer and first authored by Dr. Geethanjali Devadoss Gandhi, Senior Postdoctoral Fellow at Sidra Medicine, the study analyzed the genome sequencing data of more than 14,000 participants from QPHI, focusing on 125 genes associated with 115 treatable IMDs.

Inherited Metabolic Disorders (IMDs) are a group of rare conditions caused by enzyme deficiencies with a genetic basis that disrupt the body’s metabolism. Although each IMD is rare, their collective incidence is considerable, commonly estimated at 1 in 2,500 to 5,000 live births.

Prof. Khalid Fakhro said: “Our study fills a critical gap by focusing on the Qatari population, which has been underrepresented in previous genomic datasets. By analyzing the QPHI data, we identified 69 types of treatable IMDs within the Qatari population. Understanding the true prevalence of such metabolic disorders through genetic carrier and newborn screenings, particularly in countries with higher consanguinity rates, can offer us a detailed methodology for future investigations in population biobanks.”

The Sidra Medicine led study revealed a significant carrier frequency1 and disease burden2 for 46 treatable IMDs within the Qatari population. The results showed that approximately 1 in 13 unrelated individuals (eight percent) were carriers of at least one of these IMDs, with thirty-one participants (0.2 percent) carrying more than one disease-causing variant.

It also highlighted a concerning trend with inherited disorders like phenylketonuria (PKU)/ hyperphenylalaninemia and homocystinuria, which exhibited high carrier frequencies (1 in 68 and 1 in 85, respectively) within the Qatari population. The estimated disease burden in the population stands at 1 in 5,038 for PKU/hyperphenylalaninemia and 1 in 6,315 for homocystinuria.

Dr. Geethanjali Devadoss Gandhi said: “Our findings were particularly significant considering that data on PKU/hyperphenylalaninemia prevalence among Qataris is currently unavailable. It underscores the immediate need for further comprehensive research on treatable inherited metabolic disorders. We are hopeful that such research will also lay the ground for new treatment options to address previously unrecognized health concerns within the Qatari population.”

Dr. Ammira Al-Shabeeb Akil, Principal investigator and the lead of Genetics and Metabolic Clinical research program added: “Our study emphasizes the importance of extensive population

screenings across the MENA region. This will pave the way for precision medicine therapies tailored to specific populations. To that end, we believe that our joint Newborn Genome Screening Research Program in collaboration with Hamad Medical Corporation, will help with the early diagnosis of genetic conditions in babies in the general population in Qatar.”

The Sidra Medicine treatable IMD study was also awarded first place for ‘abstract presentation’ at the MENA Congress for Rare Diseases 2024 in Abu Dhabi, highlighting its far-reaching impact on other Arab populations.

Qatar’s leading healthcare provider selected among 22 global centers for the sunRIZE Phase 3 clinical trial

01 December 2024, Doha, Qatar – Sidra Medicine, a member of Qatar Foundation, has successfully conducted Qatar’s first industry-sponsored pharmaceutical trial for pediatric congenital hyperinsulinism on a young patient.

The trial follows Sidra Medicine’s launch of its Clinical Trials Program, aimed at offering a new lease of hope to children with rare and complex diseases. It primarily focuses on clinical trials that will benefit children in the Arab region, particularly those with rare diseases that lack effective treatment.

Sidra Medicine was selected as one of 22 leading centers worldwide to participate in the sunRIZE Phase 3 clinical trial. Led by Rezolute, a biopharmaceutical company specializing in rare diseases, the trial underscores Sidra Medicine’s growing prominence in pediatric endocrinology and its commitment to advancing innovative, patient-centered care.

Congenital hyperinsulinism is a complex condition that causes life-threatening hypoglycemia in children due to excessive insulin production. Many patients are unresponsive to existing therapies and often require invasive pancreatic surgery, which can lead to lifelong complications such as diabetes and pancreatic insufficiency.

Prof. Khalid Hussain, Division Chief of Endocrinology at Sidra Medicine, said: “This clinical trial represents a pivotal step in advancing care for congenital hyperinsulinism. Phase 3 trials compare new treatments to existing options in a diverse and larger patient population, providing evidence for regulatory approval based on effectiveness and safety. Our collaboration with Rezolute and international experts aims to develop a safer, non-invasive treatment option that could transform the lives of affected children and significantly reduce the need for surgery.”

The clinical trial was conducted on a 3-month-old Qatari girl, who was admitted to Sidra Medicine upon being diagnosed with congenital hyperinsulinism. After previous therapies to treat her condition were not successful, the child’s parents enrolled her for the SunRIZE trial as a last possible option to find an effective treatment.

“We are extremely pleased with the results of the trial as the young patient has responded favourably to the new medication. After careful monitoring to ensure she was in good health and was not experiencing any post-medication side-effects or risks, she has been discharged. Her blood sugar levels are now in the normal range, and she is feeding normally. We will continue to monitor her progress in the clinic every three months. We would like to thank the girl’s parents for putting their trust in us to find the best possible care and cure for their child,” continued Prof. Hussain.

The Congenital Hyperinsulinism trial, approved by Qatar’s Ministry of Public Health, was conducted by Sidra Medicine’s Endocrinology and Clinical Trials Program in collaboration with several internal departments, including Nursing, Pharmacy, IT, and the Institutional Review Board. Sidra Medicine is currently recruiting for more patients for the Phase 3 SunRIZE trial1, which is open to children aged between three months until 18 years of age.

Dr. Antonella Cioce, Clinical Trials Office Manager at Sidra Medicine, said: “This clinical trial highlights the exceptional teamwork and dedication at Sidra Medicine in advancing therapies for rare pediatric conditions. Families play a vital role in this journey, as their participation helps drive progress in developing innovative treatments. The trial represents a significant step toward transforming the care and outcomes for children with congenital hyperinsulinism, offering the potential to redefine treatment standards and bring hope to patients locally and globally.”

In addition to endocrine disorders such as Congenital Hyperinsulinism (CHI), Diabetes Type 1 (TD1), Sidra Medicine is also focusing on disorders and medical conditions such as neuromuscular conditions like Spinal Muscular Atrophy (SMA); Metabolic disorders like Homocystinuria (HCU) and pediatric cancers like Low Grade Glioma (pLGG).

Earlier in 2024, Sidra Medicine announced a groundbreaking protocol for treating congenital hyperinsulinism in a young Kuwaiti patient, marking another milestone in its mission to pioneer innovative treatments for rare diseases.
Qatar Breakthrough Study Sheds Light on Complexities of Recurrent Implantation Failure

24 November 2024, Doha, Qatar – Sidra Medicine, a member of Qatar Foundation, been awarded the esteemed ‘Samir Abbas Best Research Award’ for its pioneering study in reproductive medicine.

The recognition, awarded to a Qatar lead team and co-authored by a Qatari clinical embryologist, Dr. Abdulla Almohammadi, was announced during the 31st Annual Scientific Meeting of the Middle East Fertility Society (MEFS), which was held from 21 to 23 November 2024 in Doha.

Titled “The reproductive potential of vitrified-warmed euploid embryos declines following repeated uterine transfers”, the study, a collaborative effort with Hamad Medical Corporation, was published earlier this year in ‘Reproductive Biology and Endocriology’. The research sheds light on the complexities of recurrent implantation failure (RIF) in fertility treatments – a condition that imposes significant psychological and financial challenges on patients undergoing multiple in-vitro fertilization (IVF) cycles.

Dr. Abdulla Almohammadi, Clinical Embryologist at Sidra Medicine and the study’s co-author, said: “Our findings highlight the intricate nature of RIF, which can be a frustrating and challenging burden on couples – both emotionally and financially. While preimplantation genetic testing for aneuploidy (PGT-A) addresses many cases, some remain unresolved. The research will also enable clinicians to tailor treatment strategies, including addressing underlying embryonic and uterine factors, that can improve outcomes and patient well-being. Receiving the Samir Abbas Best Research Award at MEFS 2024, further validates the far-reaching aspects of our study in being able to transform lives.”

The findings of the study will also provide valuable insights to enhance patient counseling and management strategies for individuals facing RIF, including the potential to improve overall success rates and patient experiences in fertility treatments.

Prof. Johnny Awwad, Executive Chair of Women’s Services at Sidra Medicine, said: “This recognition at MEFS 2024 is a tribute to the exceptional dedication of our team. RIF being a stirring topic within the scientific community, the findings and perspective from Qatar contribute meaningful data to support clinical management and advance the understanding of this complex issue across the globe. It also underscores Qatar’s role as a leader in medical innovation and clinical research.”

The multidisciplinary RIFS research team also featured Dr. Fadi Choucair, Lead Clinical Embryologist and Dr. Lina El Taha, Clinical Fellow at Sidra Medicine; as well as Dr. Hasan Burjaq and Dr. Moza Albader from Hamad Medical Corporation. International contributions were led by Dr. Aurora Bueno Cavanillas, Director of Preventive Medicine and Public Health at the University of Granada, Spain.

MEFS 2024 brought together over 1,200 professionals from the fertility care community across the region. The conference served as a premier platform for showcasing advancements in fertility care, which fosters collaboration among physicians, embryologists, nurses, and researchers.
